- Proposed Conservatee’s personal information:
- Name: full legal name and any aliases;
- Date of birth;
- Social Security number;
- Residence: mailing address and phone number;
- Current whereabouts (if not currently living at residence): mailing address, facility name (if any) and phone number;
- Doctor: name, mailing address and phone number of the proposed Conservatee’s treating doctor and Patient Number of the proposed Conservatee;
- Dementia medications.
- Proposed Conservator’s information:
- Name: full legal name and any aliases;
- Date of birth;
- Social Security number;
- Copy of California driver license or ID.
- Names, mailing addresses and ages (if over 18 they may be simply described as “adult”) of relatives in the “first and second degree”. They are:
- Spouse or Domestic Partner;
- Parents;
- Children;
- Siblings (brothers and sisters);
- Grandparents;
- Grandchildren.
- If there are no living relatives in the “first or second degree”, then the relatives in the “third degree” must receive notice. They are:
- Great Grandparents;
- Uncles and Aunts;
- Nephews and Nieces;
- Great Grandchildren.
Proposed Conservatee’s financial information (Please bring copies of any signed original documents that you may have, we may need to retain the copies for reference while we work on your case and we would prefer that you safeguard the originals):
- Real property: estimated value and address of any real property owned;
- Income: estimated annual income from real property, pensions, Social Security, wages, public assistance, dividends, interest, etc.;
- Other asset information, such as: bank statements, brokerage account statements, annuities, savings bonds, stock certificates, etc.;
- Financial planning documents, which might include: will, power of attorney for finance, power of attorney for health care (sometimes referred to as a “living will” or “personal healthcare directive”), and any trust documents.
